You will need a tube about 400x400

Open a new transparent canvas 500x500 and flood fill with a colour of your choice.

Add black to the fill box of the material palette.

With preset shape tool select a star shape of your choice and make the star to almost fit the canvas

Layer -covert to raster.

Effects-distortion effects-ripple-settings-horizontal 80-vertical 50-amplitude 70-wavelength 10-Edge mode repeat.

Layers-merge visible and repeat the ripple effect with the same settings

Adjust-sharpness-unsharp mask on default setting.

Selections-select all. Click on the face tube and edit-copy

Click on the canvas and edit -paste into the selection.

Select none.

Image-resize-75%. Reduce the opacity to 60%

Copy and paste the tube as a new layer onto the canvas. Place the ear of the new layer over the ear of the one the canvas already.

Add a drop shadow-setting-default but change the vertical 10 to -10.

Layer duplicate-then layers merge visible.

Effects-image effects-offset on default setting.

Image-add border-size 20 -colour white-symetric ticked.

Image add border -size 10 -black and symetric

Add a new layer and any decoration of your choice I added a single flower with the same drop shadow as before

Then add a layer for your text.

Layers -merge visible and resize to how you want the tag to be.
